As part of the government guidance we are required to keep a record of all attendees at our venues. To make the collection and recording of this data as safe as possible and to eliminate the need for pens and paper, the club have developed an online form that can be completed by parents on arrival at any of our sessions.
The form can be accessed via our website using a smartphone or tablet. Simply click on the link in the Covid-19 Test & Trace Register section, complete the details requested and press submit. Once received the form will be automatically date and time stamped and the information retained for a period of 21 days. In the event of that anybody attending a session subsequently test positive for COVID-19, this information can be used by the authorities to quickly identify any people who may have potentially come in to contact with the virus.
To ensure the accuracy of the data received please only complete a form whilst at the location. If you do not have access to a smartphone or tablet then please let one of the managers know on arrival and they can complete the information on your behalf.
